If you are expecting a new child or looking to establish care please follow ALL the steps below to establish care. We must have ALL these steps completed prior to making your appointment.
Step1: Do we take your insurance? Please click here to review our insurance information. If you do not have insurance and wish to be self pay, please note that full payment for services are due on the date of service.
Step 2: Do you vaccinate your children/child? We do require all our patients to be vaccinated. Please review our vaccine policy. We also need a copy of your child's vaccine record for their file.
Step 3: Complete the new patient paperwork.
Step 4: Complete the release of information form.
Step 5: Call us to schedule!

Well Checks
Staying up to date on your child's well visits is important to keep your child healthy. Preventative care promotes healthy growth and development. During these visits we conduct a head to toe exam and discuss your child's weight/height, vision/hearing, development, nutrition, administer any needed vaccines, as well as checking in on your child's mental health. We recommend you call to schedule in advance to make sure we get you on the schedule in a timely manner. We do well visits at the intervals listed below. Sign up for our portal to get easy access to school forms such as your child's immunization form (3231) and their vision/hearing/nutrition form (3300).
Sports Physicals
If you need a sports physical, please have the sport's forms ready at the time of your visit with the medical history part completed so we can ensure there is no delay during your appointment. These forms are available on the forms page on our website for your convenience.
Acute Sick Visits
We strive to have same day appointments for sick children. If you no longer need to be seen or are running late, we ask that you call ahead of time so we can make sure we can open up the spot for another sick child. We do not overuse antibiotics nor do we routinely prescribe medications/antibiotics without a visit. We stay up to date with current guidelines and treat accordingly.
If you have an emergency, we will do our best to get your child seen immediately or direct you to the appropriate facility.
ADHD Diagnosis and Medication Management
We can screen for ADD/ADHD after the age of 6 years. If your child has been diagnosed by an outside physician, we must have those records at the time of your child's visit to be able to prescribe any of the medication. All med check patients are required to be seen every 30-90 days in order to get prescription refills.
For initial consults for screening and diagnosis, please visit our forms page and complete the ADHD assessment forms prior to the appointment. We must have both the parent and teacher forms. The teacher forms should come directly from the school. Teachers can fax the completed forms to us at 770-513-3545.
Asthma
We are trained in the diagnosis and management of most asthmatics. For severe asthmatics, we may recommend you consult with pulmonology. We do require asthma check appointments on a regular basis (every 3-6 months) depending on your child's control.
Behavior and Mental Health
If you are concerned about your child's behaviors or mental health we are here for you. If there is any concern for suicidal ideation or thoughts please call 911 or the mental health crisis line 988. We have lots of educational resources on our patient education page.

Late, Late Cancellation, or No Show Fee
If you need to cancel or change an existing appointment please call at least 2 hours prior to your scheduled appointment to avoid a No Show/Late Cancellation service fee of $30.00 per child.
Sick Complaints at a Well Child Check-Up
Please note that your insurance covers preventative care as a bundled service. If you come in for a check-up and your child is sick, or you'd like to address a chronic issue, we are obligated to file a separate visit code with your insurance. As such, your regular copay, deductible, and/or co-insurance may apply. If you have any questions about a bill, please contact your insurance first.
